Eligibility

Who Qualifies?

Our clinical team works with your physician to confirm eligibility. You may qualify if you or your loved one has been diagnosed with any of the conditions below.

Diagnosed with Type 1 or Type 2 diabetes mellitus

A1C above 8.0 percent at most recent lab draw or recurrent hypoglycemic episodes

Currently prescribed insulin, metformin, or other glucose lowering medications

Medicare or Medicaid beneficiary with at least two chronic conditions

What to Expect
